Osage County community health survey available

The Osage County Community Partnership Board has taken the leadership role in a community health improvement process for Osage County.

This process is extremely timely and a trained assessment team of coalition members will push out a countywide assessment for the Mobilizing Action through Planning and Partnerships process (MAPP).

The Osage County Community Health Survey is a short assessment to help identify issues needing addressed in our community. We urge all members of our community to fill out this survey; even if you only work or go to school here your input is important. The survey can be accessed online at https://www.surveymonkey.com/r/OsageCommunitySurvey2016, or a hardcopy can be provided and returned by email at andrewg@health.ok.gov or picked up at Osage County Health Department and returned or faxed to 918-287-2380.

Please support and participate, or continue to, with this county-wide health improvement process as we move forward to make Osage County a healthier place for all residents, employees, students, and visitors.
